| We at Lakes Area Urology would like to make your first visit to our office as pleasant, relaxed, and expedient as possible. Our office is smaller compared to many of the medical facilities you may have visited in the past, so you will likely feel more comfortable and at ease. When you arrive our receptionist, Mary or Laura will greet you at the front desk. They will ask you to complete some information forms and ask you if you had any X-rays or blood tests performed by your referring physician. To aid us in these efforts it is important for you to obtain as much information about yourself and your problem before your visit. Prior to your visit if you know the results of any of your blood tests or X-ray reports please bring that information with you. We will do what we can to assist in obtaining this data. It will save you time at our office if you also know when and where your studies were performed and why your Doctor is referring you to see the urologist. A list of any allergies or medications and then dosages will be requested. If you have access to the Internet and the World Wide Web (www) then you could print and fill out the forms at home before your visit. The forms can be found on the left hand side of this page under First Visit Forms or under the top menu. We may ask you to fill out additional forms to help us better understand a specific problem bothering you. The forms for these questionnaires are available on our web site under the heading " First visit Forms". It will likely be necessary to obtain a urine specimen at the time of your visit. . We will try to complete most of the evaluation of your problem at the time of your first visit. If you think other tests or studies may be necessary in the office plan for some extra time. This is because we will try to perform much of your evaluation on your first visit. We apologize in advance for any inconvenience this may cause but it may save you another trip back to the office.
