1. You must take the sample to St. Joseph Medical Center laboratory in Brainerd, MN 56401 between 8:00-12:00 AM.

2. No sexual intercourse or ejaculation for 3 but not longer then 5 days before collection of specimen.

3. The Semen specimen is obtained by masturbation.

4. Semen specimen is to be collected in a clean, wide-mouth jar(it is important that none of it be lost and the total amount is collected). A 2-ounce ointment jar from your pharmacist or our office is an ideal container.

5. Specimen should be kept warm at body temperature (in coat pocket, etc.) and taken to the St. Joseph Medical Center laboratory (within one hour after collection). Please note the time of collection on the jar (or jars).

6. The specimen should be collected and taken to the lab at least three to four days before your appointment so that your doctor will have the results by the time of your appointment.
